To get this achievement simply load chapter 11 (A or B )
1.Choose the third answer every time
2. When you get to the 4th Step where they ask the name choose a different name every time!
3. Repat this process 2 times with a different name and the achievement will pop up

EDIT: When I did It for the second time I finished chapter 11 by going to the archive and reading the Documents in the top left drawer about Amara, and end up in the graveyard where the game auto saves the progress!
