How to unlock the The Cellar achievement in The Town of Light - Definitive Guide

At the beginning of the game, after going through the gate and heading up the road, go left through the gap in the emergency fencing. Follow the wall on your right until you see a door in the wall. Go in, go down the stairs.

Please note that this achievement is located outside the asylum building on the grounds below the front doors. It is in a storage room that is built in to the retaining wall.

I'm going to make a note of my idiocy here, just in case anyone else does what I did. I read follow the wall on your right and was searching around the actual Asylum wall for 20 minutes trying to find the darn thing.

Its in the rock wall that is straight ahead from the gap in the emergency fencing and on the right. There. Now, hopefully there's no one else as foolish as I am. But if there is, this will help them.

When the game begins, go forward through the gate by pressing cn_X. When you see the Asylum up ahead, there will be a gap in the fence on your left - go through it.

Hug the wall on the right, and press cn_X to go through a little door before you reach the staircase.

Press cn_S to turn on your flashlight as there is no power. After you enter, there will be a piece of paper you can pick up by using cn_X and read. Behind the little wall the paper was on, is a staircase. Go all the way down it and you will unlock the trophy :)

You can get it at the very beginning of the game up to chapter 12 inclusive. Starting the game we walk along the road, opening the gate. Soon there will be a turn on the left towards the hospital. Turning left, we go not to the entrance to the hospital, which is located on the right, but to the area adjacent to the hospital on the left. Having walked along it a little, we will come across a staircase leading down: we go down it. Next we go along the wall and look for the door. Having found it, we open it and enter. We go down the stairs into the room and get the achievement.
