This is copied and pasted from xboxachievements.com I take no credit for this solution, but I found the other ones hard to follow and so I pasted this one here to help anyone that might be confused like I was or like a different way of putting things. The video helped me out greatly, hope it helps you too :)

Chapter 1 - Ground floor - Room 3 (to the right on the shelf when you enter the room)

Chapter 3 - Ground floor - Corridor 8: Observation Ward - Room 13 (just as you enter, on a bench to your right)

Chapter 4 - Upper floor - Corridor 2 - Surgery B (on the bench to the left when you enter)

Chapter 6 - Charcot Asylum garden - On a stone bench (this will be on the fourth one you come across, on the right)

Chapter 6 - Charcot Asylum Kitchens - As you come in from the gardens, go through the big room and left at the end and into the leftmost room (on a mattress in the corner)

Chapter 7 - Ground floor - Corridor 7: Tranquil Ward - Room 9 (this is the room at the end of the corridor. The entry will be on the table in front of you when you enter)

Chapter 9 - Upper floor - Corridor 6: Semi-agitated Ward - Second room on the right (this will be on a shelf next to the window)

Chapter 9 - Upper floor - Corridor 8: In-patients > Room 10 (on the desk to your left when you enter)
