There are only 4 collectables in this episode:
The first 2 are in the cave where you have to find a way to cross the river.
1st: Head past AJ to find the Toy on the ground between AJ and a walker lying next to the river.
2nd: Go past Tenn and stick close to the wall heading to a dead end with a huge log you will find a Crystal on the ground before you get to the log.
Next is James mask, it's kinda story related and depending on how you did Ep 3 you can get it automatically at the start. This may not be needed as the achievement seem's a bit buggy but here's how to get it
Last when you are AJ in the fishing shack go a little to your right and on the other side of the oven is a half of a Skull on the ground.

Unlocked for me only getting 3 of 4 collectibles so either James mask isn't required based on certain choices, or i got lucky? also the achievement for placing all collectibles popped straight away after placing down the 1st collectible.

I didn't get so lucky with this one had to do a 2nd play through and i only had 3 of the 4 but the on Home at last it popped by just placing 1.

In this episode there are 4 collectibles however, for James’s Mask there are two possible scenarios of obtaining it that are dependent on the choice that you made at the end of episode three. SPOILER WARNING!!
James Mask Scenario 1
The Crystal and Toy collectibles can be found inside the cave that you go to. The Crystal will be on the left side of the cave near the water and the Toy is on the right side of the cave next to where AJ is standing.
James Mask Scenario 2
The Skull is inside the fishing hut when you are AJ, and it’s on your right side next to the broken cooker.
At the very end of the game the Mask can be placed on the coat hanger, the Skull is placed on the shelf on the left side of the room. The Toy can be placed on the right bunk bed and the Crystal can be placed on the desk on the right side of the room.

Just thought that I would mention that I unlocked this achievement even though I didn't obtain James' mask (I know this for sure as I didn't complete the requirements and wasn't able to place the collectable), i.e. it should be flagged as Buggy+. So if like me you accidentally miss the collectable I would recommend just carrying on and obtaining the others, rather than starting from scratch.

There are a total of four collectibles to be found in this episode:
- Act 1 - after the action in the beginning, you'll be back in control of Clem inside a cave. Walk forward to the stream and follow it left to find a crystal on the ground.
- Act 1 - in the same cave as above, head off to the side through an opening to where AJ is standing, then head to the stream. Follow it right to find a toy on the ground.
- Act 1 - after crossing the stream inside the cave, you'll get into an argument with James and AJ. After a few dialogue choices, the argument will continue and James will mention his philosophy. Make sure to choose the dialogue option "The way you think is dangerous" to get James's mask at the end of the scene. Note that if James is not with you in this episode, you'll automatically get his mask at the beginning of the episode.
- Act 3 - after the opening action, you'll be at a river trying to spear fish, then you'll head inside the cabin. Once inside, turn right to see an oven. On the ground on the left of the oven is a skull to pick up.
