You will need to find away out of the morgue. Search the nearby cupboards until you find the rib spreader. Go up the ramp and use it on the doors to the lift / elevator.
You then have a decision to make.
Go back to the doors and finish opening them. Go up the ladder and after a brief conversation on the roof of the hospital, you will get the achievement.

If you make the decision to do the former, that is indeed one of the most gruesome things I have ever had to sit through. Wow...this game sometimes.

Once you bust through the elevator and climb all the way to the roof. You will see all the Walkers around town. This will unlock the trophy.
