Once you are out of the hotel with Clementine, keep moving forward slowly. Clementine will pause and you will faint.
You will immediately get the achievement.

When you leave the hotel, you will slowly make your way through the Walkers. Clementine will stop and see that her parents are Walkers. Shortly after you will pass out. This ends the chapter and your trophy will pop.
