Enter the hotel room and talk to the man. After a fairly lengthy conversation you will fight with him. Follow the prompts and make sure you kill him.
As you go to leave the room with Clementine, a walker comes in, but ignores you be cause of all the blood and guts you are covered in. Kill it, then cut it open and smear Clementine with its guts.
You get the achievement when you leave the hotel.

Anyone have trouble getting this to unlock? I completed the entire chapter and got all other achievements, but for some reason this one is still locked. I've had achievements pop at weird times while playing this digital version, but this is new.

Played through it one more time using the rewind feature, choosing different dialogue options. I still killed dude at the end, and looked in the bag again. For whatever reason the achievement popped this time around.
