This one has uncertain requirements. For the PC version, you weren't allowed to skip dialogue, skip walking animations, skip cutscenes or use the hotspot button. I did all of those except for skipping cutscenes and still got the trophy. I also did this with the speed turned all the way up (options menu) and played the entire game in one sitting.
To be safe, play the game in one sitting and don't skip the cutscenes. That should unlock this trophy after your first playthrough. Please let me know if you can confirm the exact requirements for this trophy.
