NOTE: The wording for this achievement is a little off.
You can do this during the Prologue. You must brew 5 potions of the same kind at once. Keep collecting herbs until you are around the part where you have to sneak into the monastery (Click LS to scan the area for herbs and items). You should have enough materials saved up at this point.
When you meditate (LB + A) and select alchemy, look through the different potions and press Y to see how many potions it will allow you to make. If the number of available potions is 5 or more, brew at least 5 of them at once and the achievement will unlock.
