When chapter 1 starts, you will follow the river to the town of Flotsam. When you get there, you will have to go through several cut scenes. When they are over, you can explore the town.
There are 2 dwarves in an alley working at a forge in the outside part of town (before you enter the town square where the execution scene was). Talk to them and they will craft items for you. At the start of the chapter you will have found a recipe for a new silver sword, buy the materials you need from the dwarf then tell him you want to craft an item. Press Y to craft your sword and get the achievement.

Seems pretty important to mention the long fight scene on the path to the town... And you have to stay in the mage's bubble to avoid arrows

You can get this achievement during Chapter 1. Once you reach Flotsam, head to the Non-Human District. Here you will find two dwarves in an alley working at a forge. You should already have the recipe for a silver sword. Just buy the necessary ingredients from the dwarf and ask him to forge the item for you. Once done the achievement should unlock.
