This achievement can be missed and takes place at the end of Chapter 3. Create a save after the dragon attacks the amphitheater and flies away with Sila. After climbing the tower and making your choice to save Sila or let her die (each with their own achievements), you will have a final battle with the Saskia, the dragon. After defeating the dragon, you are presented with a QTE that results in the dragon crashing into the ground and getting impaled on a tree. When presented with the choice to kill the dragon or walk away, choose to walk away, thus sparing Saskia. Choosing to kill her will grant you its own achievement, so reload the save made prior to dealing with Sila and you can choose to kill Saskia after dealing with her a second time.

The opportunity for this achievement comes towards the end of the game, after the dragon impales itself on the tree. There are two different ways to get it, depending on whether you chose Roche or Iorveth's path.
Roche's path -
Just spare the dragon and walk back to Loc Muine.
Iorveth's path -
Will automatically unlock as you progress through the later stages of the game.
