How to unlock the Threesome achievement in The Witcher 2: Assassins of Kings - Definitive Guide
To perform a group finisher you first need to upgrade your swordman skilltree and unlock the Combat Acumen. This will allow you to build an endurance meter which results in a group finishing move when you press up on the d-pad.
You can do this almost anywhere in this game as long as you have this upgrade you can perform this on any small group of enemies. When you're ready and in battle, make sure you have at least three enemies which you can finish at once. You can only perform a group finisher on three human enemies, if you perform it on creatures it will just kill the one closest to you.
This move is extremely useful if you find yourself overrun by enemies at any given time. The only down side is it takes time to build the meter.

Every time I do this on human's or monsters, I only seem to be killing one. Am I doing something wrong? Are the opponents suppose to be worn down some of positioned in a certain way?

Me as well, have 4 around me 1 dwarf 3 elves...I kill 2 elves with all 4 WELL within sword strike distance.

A great place for this is on Ioverth's path. There is a point in chapter 2 whether you sneak into a Nilfgardian camp during the main campaign. Towards the end of the quest you'll face for human enemies in a small area‚ which is perfect for this achievement.
To be able to perform a group finisher you need to upgrade unlock Combat Acumen in the Swordmanship Skill Tree (see “Swordmaster”). This will allow you to build a meter which. Once it’s full you can execute a finishing move by pressing up on the d-pad.
If you execute a finisher amidst three or more human enemies, then a cut-scene will begin showing Geralt pull off a flowing three kill attack. The achievement will then unlock. Beware, this will only work if you are surrounded by human enemies. If there is a creature in the group, Geralt will only attack the creature.
