How to unlock the A Friend in Need achievement in The Witcher 3: Wild Hunt - Complete Edition - Definitive Guide

This is a story related achievement and cannot be missed.

However, I managed to complete the associated quest and I didn't unlock the achievement. I attempted it again, and still no achievement, so I looked at a guide. Third try, I did something different and the achievement then unlocked.

This achievement should be marked as missable because of this. I now have to replay a lot to get this because I was under the impression it wasn't.

the water hag contract is very easy too, even on death march

It bugged out on me as well. I've tried it three times, following each and every step mentioned above and finding every evidence along the way to the hut, but still nothing.

Yep, same here, found the hat and notes etc, followed the correct path to find Dandelion and nothing.
