You get this achievement for completing the Level 22 Witcher contract "Mystery of the Byways Murders" that you can get in Oreton.
In the fight, I used Vampire Oil on my weapon and the Igni sign and he went down fairly quickly. You can also use Axii to stun him if you need time to heal.
**** Adding Note (below) from BegForMercy as this (like a lot of achievements in this game) appears to be bugging out for some people. Tested this for other achievements and this trick appears to work for most of the achievements that don't pop. Make sure you save often, especially before turning in quests with achievements. ******

NOTE: I had to do the hold power button down and clear network MAC and reboot trick. I had to refight the ekimmara and it pops after to talk to the villagers again. Save right before that. It does not unlock at the commander!
