The camera angles can get weird when dodging during fistfighting, but that's how it is.
The only trick here is to dodge(
) back or sideways as the enemy is making their attack move and then go towards them to attack (
) with 1 or 2 punches. It's best not to get greedy for a 3rd hit, for they might block and/or stun you and then end up hitting you instead.

This one seems buggy. I defeated the first 3 guys in white orchard, and the arsonist, all without taking damage, and it never popped.

The quickest and easiest way for me was to get this during the brawl against the Bloody Baron. You should encounter this brawl about 3 or 4 hours into the main story. The best thing about this is that its 1v1 and no guards will attack you. It's easy to beat him if you anticipate his punches by dodging out of the way, then hit a couple back on him, or try to land a heavy punch. If you do get hit, you can reload a save and it should bring you back to the start of the fight.
