Not a solution, a question: Is this achivement STILL glitched after 3457 Patches and still IN the GOTY Version? It's really stupid. I did everything in the right order. Nothing. Help anyone? Thnx
EDIT: Ok it IS glitched like before.
Solution: Clear the Xbox One cache with a hardreset (hold the powerbutton on the console pressed till it goes off) or clear the MAC adress under settings > network > advanced network settings (i did both).
Restart and do the final conversation with Keira Metz again and send her to your homebase in Morhem. Got it this way. Good luck!

I slept with her but ended up killing her on the island - but the achievement then popped. Didn't like that outcome though so reloaded a checkpoint and did it differently.

Thank you for the solution! I was getting the same problem for other achievements not unlocking such as kill 2 contract monsters with a certain requirement. I only cleared my MAC address, loaded my save for keira quest and got the cheevo. Cheers :D

This is a missable Trophy in itself, and by not doing this will also cause you to miss out on Full Crew - Bring all possible allies to Kaer Morhen for the battle against the Hunt. (Secret)
To set things in motion, you need to complete the Main Quest "Hunting a Witch", this introduces you to Keira.
Another Main Quest opens thereafter, "Wandering in the Dark". Upon completion, talking to Keira will give you the option for Secondary Quest "Magic Lamp".
To unlock this Trophy, agree to help/find/goto/fetch/assist in everything!
After "Magic Lamp", you will be offered "An Invitation from Keira Metz" - this consists of 3 smaller quests (A Towerful of Mice, A Favor for a Friend and For the Advancement of Learning).
If you have agreed to all her demands, you can talk to her and suggest another way to keep her safe - propose to send her to Kaer Morhen,
Keira's sub-plot is then marked as complete, your Trophy will unlock and progress toward Full Crew is also made.
