There isn't a quick way to do this, and you'll most likely unlock this by playing the game. For example, I unlocked it during the final quest in the main story, without aiming to unlock it.

I tracked this using the world maps of Velen/Novigrad, White Orchard & Skeligrege(sp?) as there is counter in the map legend. I got this just by wondering around and doing everything I could at whatever level I was at.

This didn't unlock for me. Fortunately I had a save before hitting 100 sign posts so after an Xbox & game reboot I found the 100th again and it dinged.
