Couple tips:
1) Save before you buy the bolts, theyre expensive
2) turn on “enemy upscaling” and play on “story and sword” difficulty
I was level 53 and the dudes had more than enough health to absorb the damage

IMPORTANT: You absolutely have to use BROADHEAD bolts, and not TRACKING bolts!
The tracking bolts description also mentions a "100% bleeding chance", but for some reason, it doesn't count as such towards this achievement. Learned this the hard way trying to unlock this achievement with tracking bolts for like an hour or so...

Easy, although in this GotY edition you can't use the Xbox Dashboard thing to keep track of how many you've successfully performed, unfortunately.
That's okay though for if you do what I'm about to suggest you will end up unlocking the achievement just like I did.
I've mentioned in other solutions about the great TIR TOCHAIR BLADE. One other use for it is to unlock this. You must attach 2 superior Morana runes to it (+5% chance each to cause poisoning). That way you'll have a single blade that will cause 2 out of the 3 effects needed. Keep in mind though that (I think) the effects won't be cause automatically (since it's a "chance") nor at the same time/hit. When I was going for this I made sure that the little words "bleeding" and "poisoned" popped up above the enemies, that way I knew they were under the respective effects.
The third effect, incineration, is the easiest of the 3, imo, because there's so much you can do to increase your Igni's power. For instance, I was using an armor that gave +20% Igni intensity and then I attached to it a Superior Igni Gliph (another 10%). I also had two other such gliphs in other parts of my armor set beyond other stuff that intensifies the sign or the chance to ignite enemies on fire (Melt Armor + Boosts of blue mutagen + an Igni Place of Power activated with Gorged on Power...).
Have in mind though that you don't want to have a very high DPS/attack since your intention is not to kill enemies but put them under those 3 effects at the same time. Eventually they'll die, sure, but you don't want it to be super fast before you're able to trigger the effects.
It's also important to notice that you can/should use the TIR TOCHAIR blade even against monsters. Not all of them bleed or can get poisoned (such as ghosts and drowners) but you'd be surprised to the variety of monster who can get under those effects. After you hit them and they are bleeding AND poisoned, then you should use the Igni sign and hope for a incineration. As soon as the incineration happens you can hit the enemy with a sword to make it stop and try to pull it all off again if the enemy still have enough HP.
Even though I couldn't keep track of how many successful "overkills" I had performed, when the achievement popped up I wasn't expecting it. I used every chance I had to try this method and it unlocked sooner than I'd expected.

The latest version of the game turns the Devil's Pit into a settlement, so look for the two respawning pirates in Skellige, as shown in the video.
For me the following combo worked:
- Devil's Puffball for poison
- Igni with Pyromaniac for burning
- Broadhead bolt for bleeding
Just rest and they will respawn, the cloud from the grenade should still be there, so if you see the pirates already losing health you can just skip to Igni. Turn the difficulty up if they die too fast.
