The Witcher 3: Wild Hunt - Complete Edition

After the battle with Eredin and a scene at the tower with Ciri, you'll get a quest called 'Something Ends, Something Begins'. Follow the objectives (which are different depending on what choices you've made with Ciri) and you'll receive the achievement after the final cutscene, regardless of which difficulty you're on.
