You will need to unlock the "Puppet" skill from the Sign tree and equip it in your skills. This is one of the best Signs to use in Death March difficulty, as it makes your opponents fight each other.
Once you have it equipped, find a group of enemies and weaken one enemy, then cast the alternative Axii sign (hold
for a split second and release instead of tapping) on the other enemy. Keep casting it until you have made an opponent kill another opponent 20 times, at which point, the achievement will unlock.... Good luck! (Best place to do this is in Devil's Pit in Velen)

For another grinding method: Go to gameplay options, turn on manual sword sheathing, turn difficulty down to just the story. Go to the spot with the two infinitely respawning pirates in Skellige, quick punch one, then cast this on the other. He’ll kill the other pirate instantly. Draw your sword to finish him off, then run away and wait for them to respawn, sheathing your sword again. My Xbox needed a power cycle for this one to unlock just fyi!
