Provoke all Iris von Everec's Nightmares into fighting you at the same time and defeat them.

How to unlock the When It's Many Against One… achievement in The Witcher 3: Wild Hunt - Complete Edition - Definitive Guide

This achievement is missable.

You can unlock this during the main quest Scenes From a Marriage.

After repairing all of Iris von Everec's memories, you will come to a large room with a fireplace. Interact with the piece of paper on the table in front of it to start a cutscene. Once this cutscene has finished, you will be thrown straight into combat with Iris von Everec's Nightmares.

There are a total of 6 enemies here, but 5 of them are inactive to begin with. For this achievement, you must fight all 6 at the same time and defeat them. To wake them, I recommend using the crossbow and manually aiming at them as Geralt cannot automatically lock onto the Nightmares with his sword or Signs. Double check that you have each enemy coming towards you before you begin to fight them.

Once you have defeated the Nightmares, this achievement will unlock.

My advice:
Attack, roll, repeat.
Protect yourself using the Quen Sign.
Use the Northern Wind bomb to freeze your opponents then use a strong attack to strike them as a group while they are unable to move.

NOTE: There is another missable achievement tied to this quest that I have linked below.

Just wanted to add the Achievement didn't unlock for me immediately after defeating the nightmares as stated in the guide but a bit into the following cutscene/conversation. May be a rare occurrence, but I still thought I'd add it for future followers of the guide.

If you are playing on death march I recommend using the the cluster bombs to wake up all the images

Another great way to wake all Nightmares is using the radius Aard attack (the big blast so to speak). That way you blast 2 or 3 at the same time to wake them up.

I came here specifically to see if the solution recommended using the crossbow to activate the five specters that are initially inactive. I reloaded three times (plus the initial attempt) trying to activate them all with the sword before I was like, I have a manually aimed ranged weapon to do that with. Definitely the easier way to begin completing this. Thank you for including it, as I’m sure it will help other people who don’t consider it on their own.

This didn't unlock for me even though I followed the guide. Guess I have to reload an earlier save and try again.

:Edit: this unlocked me when I made a selection during the conversation/cutscene that follows.

This isn't unlocking for me. At all. Followed a powerpyx guide and also another where to use aard to wake them but it's not unlocking. Replayed it 6 times, not popped.

Update tried again today after I calmed tf down. Swapped to dancing star bombs, pretty sure all the times I did do it had 6 ghosts but I did notice that I only had 5 on one go so I replayed the checkpoint. Did exactly the same as I've been doing yesterday, hit them in a cluster with a dancing star and then wailed on them cheevo popped. So for me meeting the cheevo criteria wasn't hard. Getting the cheevo to pop is the hard thing about it. Thank god that's done.

Northern Wind bomb does not work. Use firebombs instead

Bom dia, alguém aí sabe me dizer se eu diminuir a dificuldade para a luta e depois que desbloquear a conquista eu voltar o save anterior, a conquista de terminar o jogo no Marcha da Morte fica inacessível?

Using bombs was the key for me. I couldn't seem to get the guy to target different enemies with the sword properly in this fight.

Really struggled with this even on normal difficulty, I was always near death by the time I'd triggered all 6 enemies to fight. So I dropped the difficulty to easy and it was then an absolute breeze, a few bombs and a few slashes and it was all over.

So if you just want the achievement, changing the difficulty is the simplest way to go - if you're on a deathmarch playthrough, if you tackle this after finishing the main story it won't matter anyway, or you could just drop the difficulty to get the achievement and then reload an earlier checkpoint, do the fight again on your favoured difficulty level and then you don't need to fight them all at once.
