Why the negative vote?
There is a random encounter where a bunch of bandits are done ravaging a village. The village is called "Toderas" and it is located south-east from the "Hanged Man's Tree" (this tree is the first place you arrive to in this area). It is located east of the
Toderas alone:

So, how is it done? You need to have crafted the grapeshot bomb (doesn't need to be the enhanced version), go to this place and choose to fight the bandits (choose the "Actually see several monsters..." dialogue option). Once the fight starts, quickly make a jump back (+) to get a bit far from the bandits and therefore, away from the blast radius of the grapeshot, and then throw a grapeshot right in the middle of all the bandits. Most of them, (or all) should blow up in pieces.

Might be a *minor spoiler* here.
In "Now or Never" (I think it might be a side-quest, hardly missable though ) once you're leading Triss out of the sewers there's a point where you have to fight off some rats. Well, they seem to count as enemies and there's loads of them coming at Triss, so a simple Igni in their faces would net you the cheevo as long as you'll remember to put the Quen habit on hold.
You will be attacked by a major monster (did not specified which one to avoid spoiling it) and a few drowners + 1 water hag. What makes it easier to do it here is that most of the monsters will fight the people that are also involved in the quest (Bloody Baron's squad and /or Church of the Eternal Fire - i don't know if it depends on what your choices are), so just keep close to the fight and last hit any low hp enemy without using quen sign or getting hit.
A great place to do this is at the Hanged Man's Tree Fast Travel Point on the Velen Map. That's the point in the middle of the map where you enter Velen for the very first time. Near the fast travel point, there's a pack of level 1 wolves. Block their attacks until they are all in front of you, then use the Igni sign to kill them instantly.
When holding down the block-button () they cannot hurt you. If there are only a few wolves left you can take them out with some quick attacks.
Also be sure to create a save game before attempting this. In case you fail you can reload your save game and try again. You can also make the wolves respawn by fast traveling to another map (away from Velen).

You can do everything with the same wolves at the hanged tree - we leisurely stand in the block and wait for the “window” in attacks, and calmly fire Igni, three such times were enough for me to kill 5 wolves.
