How to unlock the Last Action Hero achievement in The Witcher 3: Wild Hunt - Definitive Guide
This solution contains story-related spoilers.
This achievement is unlocked at the very end of the story. The first step is to make a manual save when you reach the "Night of the Long Fangs" main quest. At the beginning of this quest, you can decide to either go to Orianna who will reveal the Unseen Elder's location, or go to Damien who will tell you Syanna's location. If you choose to find Syanna, you must talk to the girl with flint and take the ribbon from her when you enter the Fablesphere during the quest "Beyond Hill And Dale" (the Fablesphere is the dream world with the fictional characters in). This is an optional objective. If you do not complete that objective, it will result in Syanna being killed by Dettlaff, which voids the achievement. Alternatively, if you selected the Unseen Elder path during Night of the Long Fangs quest, simply proceed to the end of that quest and you will get the achievement after killing Dettlaff and proceeding to the medal ceremony in Beauclair. It will also unlock at that point if you chose Syanna's path and she was not killed by Dettlaff.
I recommend saving at the start of Night of the Long Fangs quest so you can reload later and choose the other path. The Kling of the Clink achievement requires you to choose Syanna's path and to avoid talking to the girl with flint in the Fablesphere, which results in Syanna's death. It is not possible to get both achievements in a single run. If you do not have an earlier save to return to, you would need to play through the entire DLC again from the beginning.
I got the Klink achievement after talking to her and not taking the ribbon, haven't tested reloading and taking the ribbon yet though.
Though, to be fair, I did make a save at "Night of the Long Fangs" anyway. Just so I can play the other ending... :)
After the "Capture the Castle" main quest, you start "The Night of Longfangs" quest. This quest has a choice in it. You can choose to find Syanna or you can choose to look for the Unseen Elder. You can get this achievement through either path. If you follow the Unseen Elder path you just need to complete it to the end.
If you choose the Syanna path, then when searching for Syanna, you'll end up in the Fablesphere. The main quest within the Fablesphere ("Beyond Hill and Dale") has a number of parts to it that can be done in any order. One part in particular is to talk to the Flint Girl. This is the key task for getting this achievement. The Flint Girl has a ribbon which used to belong to Syanna. In order to get the achievement, you need to get the ribbon. You can either buy it or play a round of Gwent for it.
Having gotten the ribbon, proceed with the rest of the quest and you'll leave the Fablesphere, set up a series of cut-scenes with minimal interaction, followed by a really tough battle against Dettlaff.
Regardless of which path you took, after defeating Dettlaff, just follow the quest path to the awards ceremony (optionally taking some minor detours for a bit more detective work and background) and the achievement will unlock at the ceremony.
While this is basically the end of the DLC, note that there is quite a bit of extended conversations and minor tasks to flesh out the full ending after the achievement unlocks.
Note that you cannot get this achievement and
at the same time. If you want to get both with minimal replay, choose Syanna's path and make a manual save near the Flint Girl. Get or avoid the ribbon, finish the game, then reload and make the other choice with the ribbon and go from there.
Story related but can be missed! Make sure to make a backup save at the main quest "The Night of Long Fangs".
This can be accomplished in two ways. One is during the quest of "The Night of Long Fangs" where you need to make a choice to either find the Unseen Elder or find where Syanna is being kept. Choosing to find the Unseen Elder, you can just go through doing the quest until the end. If you choose to find where Syanna is, you will come across a quest "Beyond Hill and Dale". During this quest you will have the option to speak to the Little Flint Girl. Do so and get the ribbon from her and then finish out the story. Either way, after you beat the game, you will be decorated by the duchess and the trophy will be yours.
During the "Beyond Hill and Dale..." quest, speak to the little girl selling flints and either buy for 500 or win the ribbon in gwent. Once you give the ribbon to Syanna plant the beans and finish the quest. After "Beyond Hill and Dale..." is done finish Tesham Mutna (No explaination or spoiler needed) and then "Pomp and Strange Circumstance" starts. You have two choices in this quest either do an investigation or go directly to the ceremony. For the trophy you can go directly to the ceremony and the trophy will unlock at the end of a cinematic.
Please refer to "Kling of the Clink" for more information.
To get it, you need to take a ribbon from the girl in the quest “A Long Time Ago” to protect Syanna. In this case, Datloff will not be able to kill her. After killing Detloff there will be a quest "Ceremony", in which you will receive a medal and, accordingly, a trophy.
