How to unlock the Family Counselor achievement in The Witcher 3: Wild Hunt - Definitive Guide
The Quest should't be any Problem, but if you wonder why you can't complate the Quest: you need to finsh the Main Quest Ladies of the Wood.
Can someone confirm the Hint by SilvrgunSuprman?

I finally got this to unlock this morning. I would say this achievement IS missable. When you have the conversation with the Baron you have to agree to go with him to the swamp; even if you don't agree the Baron will convince you to come and you kill the Fiend by the Crones hut BUT you won't get the achievement. Just keep agreeing with everything the Baron says and it should unlock.

Think this actually may be missable, still havnt unlocked for me and the questline i finished. Maybe it has to do with the choices you make that might have a negative impact on the quest giver ?

I see you have since gotten the achievement, was it just stuck on "unlocking"? I don't think this is missable.

Given for completing the quest Family Matters in Velen. To do this you need:
- agree to investigate the disappearance of the Bloody Baron’s wife and daughter;
-disenchant/kill Igosha;
because I disenchanted him, Igosha showed me the path of his family, therefore:
-follow Igosha and find out what happened to the Baron’s family;
further, the quest is divided, since the Baron's wife and daughter are also separated:
-the Baron’s daughter is in Oxenfurt, to get there you will need a travel document, which the unfortunate Baron will give us;
-to find the Baron’s wife, you need to go through the second chain of quests in Velen - “Witch Hunt”
In order not to spoil it, I will only say that at the end of the “Hunt...” you will find out the location of the Baron’s wife, of which you should immediately notify him and agree to join him in the liberation campaign, which will end in a battle with the Demon.

You can go free the baron's wife immediately after telling him where she is. And most importantly (I won’t spoil it) - BE SURE to win a unique baron card before this campaign, since then he will not play.

My trophy dropped before the quest with the “liberation campaign”, i.e. as soon as I told the baron where his wife was.
