How to unlock the Fire in the Hole achievement in The Witcher 3: Wild Hunt - Definitive Guide
Firstly, to destroy the actual nest you need a bomb (Samum, Grapeshot or Dancing star do the trick), so if you haven't found the schematic for any of these yet you won't be able to do it (I'm fairly certain the schematics for all three can be found in White Orchard).
Once you've got you bombs ready you have to kill all the monsters surrounding the nest and then press A on it when the icon comes up to drop a bomb inside of it. Stand back so your not caught in the blast radius and then the nest is gone.
Once or twice once I've blown a nest up another monster has appeared that I've had to kill but that hasn't always happened to me.
Monster nests show up as question marks (?) on the map. I have found 3 nests in White orchard and at least 7 in Velen so far, but I'm sure there are a ton that I haven't managed to find yet.
Good Luck :-)

2 - Filter by "Monster Nest" location only.
3 - All available monster nests will be indicated on the interactive map.
4 - Remember to craft enough bombs to destroy at least 10 monster nests.
5 - Choose the location where you want to start and have a good hunt.
Just in case, here's a video tutorial on the achievement. Credits are from the Channel "AlienTenno"
Although monster nests are not directly labeled on the world map, you can find them by visiting the various question marks that appear in a region after checking notice boards in cities, towns, and villages of that specific region. Once a question mark hiding a monster nest is discovered, the nest will be indicated on the mini-map and world map as a "dragon head" icon. More than likely, you will have no problem finding at least 10 monster nests, especially since the video guide for "Pest Control" already covers the location of the 5 nests in Skellige. For reference, there are around 23 more nests in Velen/Novigrad and another 3 in White Orchard.
The Grapeshot Bomb is your go-to explosive when destroying these monster nests. You can craft it in your alchemy menu at any time during your playthrough and replenish your supply with alcohol while you meditate. All in all, the monster nests must be interacted with directly so that Geralt can place the bomb near the mound to destroy it for good. Once 10 monster nests have been destroyed via bombs, this achievement will unlock.
You will get this easily enough while progressing through the game. Nests can be found in every area, by exploring the map where you see a "?" and eventually you will uncover a monster nest that will be represented by a "dragon's head" icon.
Clear out the monsters around the nest and then use a bomb (Grapeshot or Samum) to destroy the nest. The Grapeshot is your best bet as it is the easiest to craft, using 2 Saltpeter and 1 Calciumequum. After destroying 10 monster nests, the trophy will unlock.
- don’t throw bombs at them, just activate X and go away
- sometimes the nest may be empty or the fat enemy simply will not appear. In general, there is a freebie.
- and don’t forget to collect goodies after the explosion
