now after you do the race with Ciri you will be in the courtyard, select to do the combat training with Vesemir and lock on to him, now after a few moves you will learn to parry then counter attack.
after Vesemir puts a shield up and you break that shield down by casting spells, he will then ask you to cast the Aard sign, don't do this and Vesemir will just keep attacking you and you can get the 10 counters easy, just wait until he moves to attack you and press LT.
do this 10 times and the achievement unlocks.

Until I went to Skellige, picked a town and saved before I selected a guard. I removed armor and put on a level 1 and level 3 sword(s). When I could see a red skull above the aggro'd guard, then I started to block his attacks. Then after 10 consecutive blocks with a kick back of the guard; the achievement unlocked. This was last night. Good luck. If you are at a high level, I think this may work for you. I also downloaded the latest patch.
The enemy health/stamina bar will also flash red, use this as a guide as well.
Find a group of soldiers, kill all but one and then keep blocking his strikes until you unlock the Trophy.
You can also do this in the tutorial early in the game where you do the training session with Vesemir. During your combat with him, he'll have you break down a shield and then ask you to hit him with one more Aard. Instead of hitting him with the final spell, just wait and he'll continue to do simple attacks that you can counterattack.
This works best against human city guards with swords. Archers are also perfect as they always carry a sword. Look for a single city guard and counter 10 attacks in a row. You must press the block-button () just before the enemy is about to hit you. Always press the block-button when the enemy has his sword vertically in the air above his head (a fraction of a second after he starts swinging the sword and a fraction of a second before he hits you). Your timing needs to be perfect and it's going to take a few tries to complete.
The best place to do this is by the river, North of the Woesong Bridge fast travel point on the White Orchard map (first town you enter in the game). There's a friendly archer here. Attack him to start the fight. Doing so against friendly guards is great as they will only knock you out and not kill you. So if you mess up you will respawn right in front of the archer without any loading times and can keep trying. Getting knocked out will cost you money, so make a backup beforehand.
While this achievement may come naturally to some players over the course of their playthrough, there’s an easy method to follow in obtaining this. The best isolated location to quickly get this done is by the river docks in White Orchard, north of the Woesong Bridge fast travel point. Here, you’ll spot a friendly archer near the docks. Begin to attack him and he’ll draw his sword and start fighting back; at which point you can start working on the achievement.
To perform an effective counterattack, you will need to hold the left trigger (block) just before your opponent swings their swords and hits you. Your timing of the block will need to be very precise. If you mess up the fight with him, allow him to drain your health. Instead of killing you, he’ll knock you out and you’ll respawn right in front of him.
NOTE: One may opt to get this achievement during the game’s tutorial. When practicing with Vesemir, he will begin to attack you frequently after the Quen and Igni Sign demonstrations. Use this time to score ten easy counterattacks in a row.
The best place to do this is by the river, just north of the fast travel point Woesong Bridge in the White Orchard location (the first city you enter in the game), there is a friendly archer there. Attack him and start the battle. The enemy will only hit you and will not kill you, if you do not block his blow, you will simply appear a little to the right directly in front of the archer without any loading and you can try again, but they will take some money from you.
