You can clear all enemies from the sites first. This will make it easy to revisit them once you found and cleared them all.
There are six of these places in the first main area of the game, White Orchard, covering all five signs with two places of power for the Quen Sign. If you want to find where each place of power is, it is outlined in this video. Hope this helps!

Start in the southeast, right of the Abandoned Village marker. Head over to the left of the same marker for the second one. Either teleport or ride to the Sawmill marker, you'll find #3 a bit north of it. Cross the river via the bridge, pass the Mill marker and the two ones found north of it are the remaining #4 and #5. If you've cleaned the area already, you should have plenty of time left, before the first one runs out.
Just note, that for being able to get this achievement, you have to make sure, that the General perk "Gorged on Power" is deactivated, meaning not put into a perk slot. This one will make a bonus to last forever, but there can only ever be one active at a time, which is exactly not what we need for this...
This is best done in White Orchard (starting area). Places of Power are ritual stones hidden in the woods of this map. They can also be found in Velen, Novigrad, Skellige and Kaer Morhen. Interact with the quest board in White Orchard to reveal their locations as question marks on the map.
Activating the places of power will earn you a skill point, and give you a significant boost for one of your signs. You have 5 signs, so you will need to activate the 5 corresponding stones. One for each sign. The catch is that the effect only stays active for 30 minutes. That means you have to activate all stones in quick succession. Luckily, there's more than enough time that you can make use of fast travel if you wish.
In the Northern Realms, Places of Power are stone monuments marked with one of the Signs used by Geralt. Drawing magic from these stones offers an intensity boost to the Sign marked on the monument for 30 minutes. What's more, each Place of Power also grands an additional skill point. Like all undiscovered areas in the world, you'll have to uncover the question marks on the world map to find these stone monuments.
There are fifteen main Places of Power in the game (plus a few extras that are found through main quests) and they are spread out evenly (5 per region) across White Orchard, Velen/Novigrad, and the Skellige Isles. For this achievement, you only have to activate all 5 stones in one region (i.e. one stone per Sign), so it's best to get this done during the Prologue in White Orchard. The effect lasts 30 minutes, so you need to activate them as quickly as possible, but don't worry about more than enough time.
