Start a new game under custom difficulty and make sure the 'No Alerts' option is on. Will not work in the Prologue, so you will have to complete the first main story level as well.
Easiest if you start with a 700pt+ custom difficulty for that achievement with 'Zero Damage' and 'No Focus' for those achievements as well.

This is simple enough but since I didn't unlock it my first run through I created a new save on Rogue difficulty and ran through Chapter 1.
I did it in around 8 minutes but I skipped almost all non-objective loot and didn't knock anyone out.

Because I know some people are wondering, "Oh if I see a yellow eye above a guard does that mean I'm boned and have to start over?" No, look at the icon of the trophy, that's what you see when they are actively searching for someone, but they don't know it is YOU. So long as the icon above the guard's head, or a civilian, turns red, you're golden. But if you don't know, you can always open up the stages menu and check, it will tell you if you are a bad thief and have been detected.

Pretty sure you meant to say "doesn't turn red" instead of "turns red".
