How to unlock the Focus on the Tasks at Hand achievement in Thief - Definitive Guide
When you go to pick a lock, hit Y to start focus and you'll see tumblers. Go ahead and pick the lock.
Now find someone to pickpocket, hit Y, relieve them of their cash.

The easiest area to do this is on Chapter 2. HOWEVER! You cannot do this right away. You need to have your skills upgraded, notably, your dexterity.
You can pay to get upgrade points at the old hag near the start of chapter 2. They are costly, so bring lots of coin.
Here is an awesome video (credited to the amazing PowerPyx) demostrating how to get this achievement right at the beginning of chapter 2.
As of this morning I can confirm that you need Dexterity maxed out to net this trophy, as it changes the lock pick minigame when focus is active.
Focus allows you to easily spot collectibles, objects you can interact with and also slows down time. To use you focus ability you press the button. Before you can obtain this achievement you must first use two focus points to level up your Dexterity focus ability. Focus points become available after you visit the Queen of Beggars just prior to beginning Chapter 2, after you visit she will automatically award you with one focus point and the ability to purchase more from her with a donation of gold. You can go directly back to her and purchase one more point allowing you to fully upgrade dexterity. Once you have done that you just need to pick a pocket and a lock whilst in focus mode and the achievement will unlock.
You don’t have to do both during a single activation of your focus ability.
You will need to purchase Focus Upgrades for this trophy, specifically Dexterity II. You can purchase focus upgrades after completing Chapter 1 from the Queen of Beggars ( on your City map).
Once you’ve purchased them, simply activate your focus ability via before you pickpocket a guard and lockpick a door/chest. You probably won’t notice a difference when pickpocketing with focus on (you supposedly loot faster) but when you’re lockpicking, initiating focus mode grants you the ability to see “inside” the lock you’re picking.
Note that you do not have to do these two tasks consecutively; you can do it at different times. Do both tasks to unlock this trophy.

However, not everything is so simple. In order to get this trophy, you need to beat the game until the moment when Grandma the Lady of the Beggars appears.
We go to its location in the church, make a donation and get the opportunity to increase concentration skills. We pump up the second item from the top called “Dexterity” all the way, after which we get the opportunity to “look inside the locking mechanisms.”
All that remains is to rob someone and pick the lock with your concentration turned on.
