For this achievement you need the ending where you give Taras the item "Property Takeovers and Extortion 1972-1978" from your collection. There are no real 'satisfying' endings, but this one seems to be on the better side.
On day 5 Milena will return with meds and food, give them to Ruben. On Day 7 Ruben should be healthy enough to move, go to the Museum with as much stuff as you can carry. Taras will appear as your trader from now on, make sure you select the dialogue option with him everytime. He should arrive on Days: 13, 18, and 23. Scavenge, save art, stay alive, and fix the truck in the meantime
Taras will then be waiting for you at the Construction Site, do this on the night of Day 23. You are now locked in to get the achievement, but depending on if you helped the Orphans escape then the amount of days you must wait wil vary:
-If you did NOT help the Orphans escape then you will be waiting around until Day 27 for Maria to say Milena can't be found and you should evacuate on the truck.
-if you DID help the Orphans escape then Maria should arrive the next day to say Milena can't be found and you should evacuate on the truck.
