How to unlock the Miraculous recovery achievement in This War of Mine: The Little Ones - Definitive Guide

For this you need to heal someone in your group from terminally ill to slightly sick (ammended from comment).

Starting your own story with 2/3 characters on moderate usually gives you one sick character at the start of the game. This is the more difficult approach as you don't have much medicine/resources at the start.

Alternatively wait until someone gets sick later in the game so you can be better prepared.

Simply don't let them sleep in a bed or give them medicine until they become terminally ill. Stock up on some medicine beforehand so when they become terminally ill you can give them meds each day. Keep them nicely fed and let them sleep in bed during the day and night to bring them back to healthy state quicker.

Bear in mind they will likely become depressed through this process so you might have to talk to them each day to prevent them going broken and possibly committing suicide.

The achievement didn't pop for me until they were slightly sick.

I pretty much had this last night. I had it set to 80 days so I'd have lots of time. Had one person to terminally ill, but I forgot to turn the hostility to low. As soon as my person hit terminally ill, the crime wave started.

This caused a chain of events that lead to both my ill person and another dying from the constant attacks. I recommend doing this with hostility set to low. (On low, there is no crime wave)

In order to get this achievement, you need to have someone start a day listed as "Terminally Ill" and nurse them back to "Slightly Sick" at worst. You might get lucky and have someone just randomly show up at your door listed as Terminally Ill and join your house, but more than likely, you are going to need to make someone Terminally Ill. Winter helps with this, but in general, you will want to make sure that if someone is listed as "Sick," you make it worse. Do not give them any meds, herbal or otherwise, and don't let them sleep too much. They will try to stop whatever they are doing and go to bed, so watch them and don't let them. Hopefully after a day or two of this, they will have progressed to the point that they are listed as Terminally Ill.

Once you have someone listed as Terminally Ill, immediately put them on full bed rest and start giving them meds every day. The real meds are best, but herbal meds will work, too. Hopefully after the first day, they will go down to Severely Ill, then Sick the day after that. When they are down to Slightly Sick, the achievement will unlock. Do not take any risks while recovering. Make sure they are fed, make sure that they are sleeping at night and resting during the day and start each day with a dose of meds.
