Be aware that you CAN miss those characters since some are bound to story/sidequest choices, but if you clear the map for a 100% and fell decisions that are helping the characters (like killing the manticore for Eyck which is also an achievement) you should have no problem getting them.
Get into a one round fight and have them all in your Deck, when on the starting screen you WANT to have Rayla and Xavier in your Hand. Play Rayla and afterwards Xavier and give all charges to Rayla, activate her 3 times and pick one of the 3 remaining characters every time.

By using a decoy card on Reynard, you play him twice, also refreshing Rayla's order twice, allowing her to summon, Reynard, Eyck and Gascon.
You should have no issues obtaining this. You do not have to play five of the cards in one round, it can be spread out among all three rounds. Make sure you have at least five character cards in your deck.
If you want to get this out of the way, bring your cards to the minimum amount of cards needed with all the character cards available in your deck. Find a fight and then keep redrawing until you have them in your hand or until you hit your limit. Play all the ones you can and then finish the round. If you were not able to play all five, try again the next round and if you didn’t obtain any character cards, lose the round, and try again for the third. If the other character cards didn’t get drawn, restart the game and redo the fight again until you get it.
The characters are recruited throughout the game as you go. Naturally, you won't be able to unlock this achievement until chapter 2 at the earliest. You will need to add at least 5 characters to your deck and then you will need a little luck and hope that 5 or more characters are added to your hand at the start of a battle. Characters available to use towards this achievement are:
- Reynard Odo: Your most loyal advisor and will accompany you throughout the game. He can be dismissed during chapter 4 but if you do he will return a little later
- Gascon, Duke of Dogs: An adversary at the beginning of the game, but who later helps you and joins your company. He too can be dismissed along with Reynard but again will return later
- Eyck of Denesle: A character found in chapter 1, needed to unlock "For the Good Book states..." Please refer to its description for details on recruitment
- Black Rayla: A character that you will recruit during chapter 2. It isn't needed for any specific achievement; however, you can have various disagreements with her and she will leave your company if you go against her wishes/advice too many times
- Xavier Lemmens: A character recruited during the beginning of chapter 2. You will lose this character towards the end of chapter 4
- The Dog: A character you can find and recruit during chapter 2, needed for "Arf! Arf! Grrr..." Please refer to its description for details on where to find him
- Gabor Zigrin: A character who will join you at the start of chapter 3 and then leave at the end of chapter 3
- Barnabas Beckenbauer: A character recruited in chapter 3, needed to unlock "Out of the sizzlixer into the fire." Please refer to its description for details on recruitment
- Isbel: A character recruited in chapter 2, needed to unlock "1. Do no harm." Please refer to its description for details
- Geralt of Rivia: Obtained during the final battle (Battle for the Bridge) of chapter 4 and will only be present for this battle. He will leave before chapter 5
There are a few more that will be largely inconsequential, but this list provides much more than five; have any five of these characters in your hand and play them in a round to unlock this achievement.
Get it automatically by playing the Assassination of the Queen puzzle.
It’s easy to get in the most common way: we put more characters into the deck, try to catch them during the card replacement phase, and, if we have the required number of characters, dump them on the table.
