In order to get this ending you have to combine two cores that don't give any of the other endings, during the dialogue after defeating the final boss:
There are a few combinations that work, but an example is Fool's God's Core + Varg's Core.
To get the Fool's God's Core you have to defeat the boss in mission Sea of Trees - God of the Fools (4/5).
To get the Varg's Core you have to defate the boss in mission Hermes Fortress - Hermes Fortress (1/3).
After combining them and getting the ending and the achievement, you will be prompted "Is this the Truth?". Select "No".
You can then defeat the final boss again and get another ending.
This can be repeated to get all endings in one save:
