This is a secret ending.
You have to follow the same steps as in the Harmonized Force ending (see steps below) but during the final cutscene you have to move your left stick around.
In order to get this ending you have to combine the Mutated Odur's Core + The Hanged Queen's Core during the dialogue after defeating the final boss:
To get the Mutated Odur's Core you have to defeat the boss in mission Sea of Trees - Mutated Odur (5/5).
To get the The Hanged Queen's Core you have to defate the boss in mission Royal Garden - Royal Garden (1/4).
After combining them and getting the ending and the achievement, you will be prompted "Is this the Truth?". Select "No".
You can then defeat the final boss again and get another ending.
This can be repeated to get all endings in one save:
