In order to get this ending you have to combine the Fool's God's Core + Sound of the Abyss' Core during the dialogue after defeating the final boss:
To get the Fool's God's Core you have to defeat the boss in mission Sea of Trees - God of the Fools (4/5).
To get the Sound of the Abyss' Core you have to defate the boss in mission Royal Garden - Underground Laboratory (3/4).
After combining them and getting the ending and the achievement, you will be prompted "Is this the Truth?". Select "No".
You can then defeat the final boss again and get another ending.
This can be repeated to get all endings in one save:
