When you come across the VHS tape in Act 1 in the garage you need to make sure to leave it so that it can be recorded over. If you remove the tab be sure to tape it afterward. If not, when you come across the tape near the end of Act 2 there will be no VHS camera to play the tape.
In Act 2, when you're searching for a radio in the attic, you will find the tape on the floor and the camera in box directly adjacent. Access the tape and the achievement will unlock.
