How to unlock the Reversal of Fates achievement in Tiny Tina's Wonderlands - Definitive Guide
To execute a death save you need to be downed by an enemy, then whilst you are down shoot the enemy to kill them. This will result in a death save and you being revived automatically.
Do this on a Boss or a Badass enemy for the trophy, the earlier you do this the easier it will be as enemies are lower level.

This is easily done during the opening mission "Bunkers & Badasses" when Tina spawns a Badass Skeleton for you to fight. Whittle his health down to almost nothing, let him down you then finish him off whilst you're in Save Your Soul Mode. The trophy will pop shortly after he crumbles into dust. Even if you don't go out of your way to get this trophy, it's inevitable you will eventually get it without even trying.
A Death Save is this game's equivalent of a Second Wind from the Borderlands series. So, you'll need to get downed and then kill a badass or boss to revive yourself.
There is a pretty decent chance this will happen naturally over the course of your playthrough, but it's incredibly easy to force it. You'll fight badasses and bosses routinely throughout story quests and side quests, so just hit one until it barely has any health, then just let it (or any other enemies) down you, then finish off the badass or boss to get a Death Save and unlock this achievement.
