You will unlock this whilst in a Co-Op game with a partner.
Once they have been knocked down you with have a small amount of time to revive them, go up to their body and hold SQUARE to revive.
Once revived, the trophy will unlock.

Note: If you do the co-op trophies with a friend, it is recommended you each have a headset equipped so there's no confusion about what's going on.
To revive a friend, they have to be in Save Your Soul Mode so have an enemy down them, then you can approach their prone form and press and hold
. The trophy will pop soon after they are revived. Please use the boosting thread here if you don't have 2 controllers handy: LINK

As the description says, you'll need to join a co-op game, wait for your partner to get downed, and then run up and hold 
to revive them. If playing co-op, this will assuredly happen naturally. If you just want to get this achievement quickly, have your partner take off their Ward and just let enemies shoot them until they go down (or have them shoot rockets at their feet, or shoot explosive barrels while standing next to them), then quickly revive them.
