In order to unlock the last zombie map (The Marching Dead in Operation 4: Blizzard), you either need to complete all 7 missions on 3-bullet difficulty... or pay 5 medals. You don't need to complete the 4th operation for any achievements, so whichever way you want to unlock it is up to you.

You do have to have all 4 at one time, I had one soldier die before I got the 4th character. The achievement didn't pop. However after the next wave the character repawned so I made my way to him picked him up and- ACHIEVEMENT UNLOCKED!

This achievement was quite easy for me personally. Got it on my first try. I'd just suggest to keep moving and kiting enemies. Make sure to keep backing up every time you target a zombie and don't get cornered.
I personally unlocked this while still in Operation 1: Unicorn Lair, so a good note is that you don't need to make it to Operation 4: Blizzard. It's easy enough to unlock once you've gathered 5 medals.

Unlock the zombie map in operation four with either five medals or 21 bullets. You start off with a single soldier, but after every couple of waves, you can pick up a new soldier. Have all soldiers survive until wave 10 when the fourth soldier joins you, and the achievement unlocks. Should a soldier die, keep on playing as you'll get a replacement on wave 12. This will still count.
NOTE: You can save yourself quite some time by unlocking the map with medals. But you can always do it the hard and time consuming way should you run out of medals. Credit to Caramel Cardinal and Lord Terentius for pointing out this alternative method.
