To unlock the Clan War trophy in The Division, you don't need to have four or your clan mates together in the match at the same time. You don't need to be facing a team made up of players that are all from the same clan either.
It would appear you need to be a member of a clan, and play a match where at least one member from the opposing team is also the member of a clan.
To join or create a clan, you can follow the instructions for the Strength in Number trophy:
Once you're in a clan, press
to open the map and press 
to select the conflicts tab, and select Quickplay. Once the game has loaded, the trophy will unlock if a member of the opposing team has joined a clan.

This doesn't have to be a full team of 4 from your clan. You can be the only person from your clan playing a conflict match and it will pop as you load in. See here for the Clan Recruitment thread: LINK

To unlock this achievement, you will first need to be in a clan. You can create your own after progressing far enough into the story to unlock the specific character that allows this. See the description for Strength in Numbers (20G) for details on how to create a clan.
Before joining a Conflict game for First Among Equals (10G), be sure to be in a clan of your own. Then to unlock this achievement you simply need to be against at least one other player on the opposite team that is in a clan too. Join a Conflict game, if you see none of the other team are in a clan, marked by a 3-letter tag before their Gamertag, either finish it for fun, or quit out and join another until you are up against someone in a clan. The achievement will unlock after completing the round.
