To unlock the Command and Control trophy in The Division 2, you will need to discover 20 control points. Control points are marked on your map with a red flag icon, which will appear when you are in close proximity to one.
The control points are heavily guarded and will reward you with a decent amount of XP and occasional fast travel points if you capture them.
For the Command and Control trophy, however, you won't need to capture them. Discovering 20 is enough.

Control points are indicated on the map by a red flag. You have to run near them for them to be discovered. There are loads across the map and they provide good loot and fast travel points so they are worth getting.

You will find control points in each district of the map. They will each have three control points, except for the three districts with the main settlements; they will have only two control points. You simply need to walk into them to find their location, or else be given their location by completing activities and side missions as you play the game.
