How to unlock the Gone Spelunking achievement in Tom Clancy's The Division - Definitive Guide
The collectibles in this DLC don't work like they did in the main game, where you can simply follow a map and find them all. Rather, you will get a chance at one collectible per phase of operations (so a max of three per game). During the levels, you will come to a non-combat area with a restock crate, and you'll want to open your map at this point to find a collectible now marked on it (maybe, more on this below), which will lead you to that phase's collectible.
You'll need to do a minimum of 80 phases for this achievement, but as I mentioned above, there's a chance no collectible will be there if you've already collected all of one type. The game randomly picks whether there will be an audio log or diary, and if you don't need diaries but they chose to spawn one, there actually won't be any collectible there for you to find. You should get this on your way to Rank 40 with no issue though.
Every Phase has 1 collectible: either an Audio Log or an UrbEx Diary and will only appear in "non-combat" areas, usually somewhere in the middle of mission-area (but not always).
Tip: Once you enter the non-combat area after your second fight, open up your map and the collectable should show up.
The collectables are known to be in the vicinity of Ammo crates.

All collected objects are always located in non-combat zones, near chests with ammo; sometimes such a zone can affect neighboring rooms. Usually such chests are located somewhere in the middle of the operation stage, if you reach the chest with ammo, there will definitely be a collectible item somewhere nearby. Open the map and he will be marked on it. There are rare situations when non-combat zones are located immediately at the entrance to a location, but in any case, if you find a chest with ammo, there will be collectible objects nearby.
Thus, in an operation from one stage there will be 1 item, in an operation from two stages there will be 2 and in an operation from three there will be three. Ideally, you will be able to collect all 80 items in 80 operations in one stage. However, the developers decided that this would be too easy, so as you get closer to the coveted 80 collected items, the game will start not putting them down. More precisely, she will put the wrong item that you need, so you will only have audio recordings, and on missions you will only come across pictures or vice versa. It doesn't matter if you are alone or in a team. Therefore, do not despair if when you only have a couple of items left for the trophy, in 5-7 operations you will not find a single one - this is normal. Just keep going through the operations until you collect them all.
In principle, you don’t need to make this particular trophy separately, you can go through the required number of operations, and collect all the items while you work to get 40 underground rank.
