A revive in The Division is very similar to Gears of War. If a player loses all his health, he will go down to the ground and be able to crawl around. Make your way to a friend and hold 
to bring them back to their feet. Revives in missions, free roam, and Dark Zone all count toward this. It can be done quickly with a friend by letting him be hit while you stay in cover and reviving him over and over. However, if you play the game with friends regularly it should come naturally, and will likely come while going for the Hard/Challenge mode achievements, as well as during Dark Zone encounters.

When a teammate is down, their agent will drop to their hands and feet holding their stomach. Your controller, or TV screen, will announce that an Agent is down. While in this downed state, a player can continue to crawl towards you or behind cover until the red ring around their agent runs out. If you get to them in time, hold 
to manually revive your teammate. There are abilities that can also revive your teammates, but these abilities don't always track revives correctly. It's better to manually revive and be sure you're getting credit until the trophy unlocks.
