There are total 171 Collectibles and 84 Challengeobjects in Tomb Raider.
The 10 Documents "Lara´s Journal" collect you automatically (9/10) when you use Camps during the Storyprogress. The last One in this Row get you in the Ending Scene and the Achievement No Stone Left Unturned pops up during the Endscene.
In Survivortree you have 2 Skills which help a Lot to find Collectibles. (Orienteering and Cartography)
You can set a Marker on your Map to a uncollected Item and when you use Survival Instinct ingame, you see a blue beam of light on the Location from the marked Collectible.
If you collect an Collectible, it is automatically saved. After the Story you can use fast Travel to each fast Travel Camp to collect the missing Collectibles or Challengeobjects.
The Numbers behind the linked Timelinks are the Ingame Coordinates from each Objects. In the Youtube Description you have the Timeline too.
When you Click a Item, a new Window open and the Video starts from this Time / Item. Otherwise you can watch the Videos below the Areas and click manually to the Time.
In the first Spoiler is always the Video and in the second Spoiler is the Timeline with the Collectibles, Challenges & Coordinates.

Just a warning, there are 2 in Mountain Village that CAN be glitched temporarily (they disappear even when its still on your map), the one on the little shack where you sometimes see a statue you have to light (but there is no statue in this one) and DIRECTLY left of it on the slightly raised grassy area.......just like with the "sound loss" glitch, just exit out of the game and restart

Skill up grade cartographer really helpful.
Not really a guide as much as information. It's best to leave collectibles until near the end or after the main story.
You will need all weapons and story related bow attachments to collect everything including some challenges.
The before mentioned skill upgrade will make collectibles appear on your map (not challenges)
Use fast travel to enter a location. Use your map to mark your collectible , collect and repeat.
One or two areas do not have a fast travel so if you find yourself missing a gps or challenge but all the fast travel areas are at 100%, go into your map and highlight each area on he map. In the upper right corner will be a legend that tells you what you have or don't in the highlighted area.
One last tip. If you can't set a map marker you can't get the item from that fast travel point.

This chieve popped after 10 mins of me thinking it was glitched (didn't show either, just appeared unlocked out of nowhere) maybe that happened to other ppl? I suggest staying in game for another 10-15 mins after credits and continuing.
