Here’s a very basic guide just to make sure you’re on the right track (count them as you break them):

BREAKABLE FLOORS (shown as total and how many are in each section in brackets, any that are harder to find have an explanation):

Caves: 2 (2). Encountered on main game route.
City of Vilcabamba: 14 (2+4+8). Encountered on main game route.
Lost Valley: 0
Tomb of Qualopec: 10 (1+1+8). The 8 is in the secret chamber behind the statue just before the Scion chamber. The others are on the main game route.
St Francis Folly: 1 (1). This is in the first secret chamber.
Colosseum: 0
Palace Midas: 4 (3+1). The 1 is in the secret chamber in the garden. The others are on the main game route.
Cistern: 5 (5). All at the end of the level in the checkered floor.
Tomb of Tihocan: 5 (5). All in the last secret chamber.
City of Khamoon: 0
Obelisk of Khamoon: 0
Sanctuary of the Scion: 0
Natla’s Mines: 1 (1). On top of the house.
Atlantis: 0
Great Pyramid: 9 (1+1+3+3+1). Encountered on main game route.

Vilcabamba has 14, 2+4+8. You forgot the 2 before to take the key and artefact.

The achievement pop right after breaking the panel for the 3rd secret in Great Pyramid.

Despite what the Achievement says, it seems you don't have to do this in one run. I managed to get this halfway through a second playthrough, after getting the final breakable tile I didn't get during my first playthrough.

Yes, it definitely unlocks for ones you missed on subsequent playthroughs. Missed the one in St Francis (twice!) but loaded up the level again and it popped once I found it.

I'm pretty sure you do have to break them all in one playthrough, as the achievement didn't unlock for me until I broke the floor at Great Pyramid's final secret. And I definitely broke that one in a previous playthrough.

Damn! I did a second playthrough and totally forgot about the 1 in the secret area of Midas Palace… that’s why I didn’t got the achievement 😭 Did it pop right after you break the last one or do you need to finish the game?

I just broke all of them in my NG+ playthrough, and the achievement did NOT pop. Fingers crossed that it randomly pops in a few hours, but it might just be that you can't get this one in NG+.

Edit: I'm currently working on my speedrun, and it just popped for me in the secret area in Palace Midas. I got that one in both my very first run of the game and my NG+ run, so it wasn't one that I missed before and finally found on my 3rd playthrough. From my experience and what others have said, it seems like this one just sometimes takes a while to properly register and may very well randomly pop a number of hours after it should have.
