This achievement can be completed during the 11th level, Obelisk of Khamoon.
After retrieving all 4 items from the obelisk, an underwater door will open.
You'll have to collect 3 items in the underwater corridor AND all 3 items in the last underwater room at the end of the corridor without replenish your breath bar.
Use medikit to keep going when you start loosing health from drowning.

I was able to get it using only one small medipack right before picking up the 6th item.

The first item in the water before the door opens doesn’t count. I wasted some HealthKit doing all seven instead of the six required.
