Tomb Raider Unfinished Business - Return to Egypt (Level 3)
You will enter the final area of the level after taking a long underwater passage and using a switch - some panthers will spawn in this part. On your left will be a fenced off area, head inside and go to the far right corner. By wiggling the stick in a certain way into this corner, you will clip outside. Ahead is a pool of water with the Uzi items (the currently glitched Bastet's Stash normally unlocks here). Climb out into the desert on the right side, two mummies roam this area. Directly ahead is a stone block with a large cat's face behind it, stand on the face for this achievement.
You can also run to the right into the desert until the game automatically kills you to unlock Exhausted.

Unfinished Business - Level 3 - Return to Egypt
Near the end of the level you will end up outside. All you have to do is jump out of the area and head over towards the big cat image on the floor. Achievement should pop while you stand on the pillar next to it. I made a short clip to show you the location.
