At the main flight of stairs, take the left hand stairwell (as you look from the entrance hall) up to the penultimate step, before it goes on to join the other stairwell.

Walk Lara backwards towards the wall, and aim diagonally for the banister. Run, then hold JUMP and WALK to Swan dive over the banister. You're aiming to land on one of the crates. If she lands and rolls from this height, your achievement will unlock.

This can be done anywhere in the game where you can find the right height - it's one full block high, plus a step (roughly a third of a block). Using the stairs gives you the exact height, and you don't have to worry about progressing through the levels to find somewhere.

It's unclear exactly what is considered "maximum height," but I got this in Peru right at the beginning on accident. If you go up to the first platform on the left as you enter the cave, turn around, and swan dive off there, it should unlock. Or at least it did for me. Again, YYMV, and I was using Modern Controls, not sure if that matters, but this method definitely worked for me. Wasn't even trying to get it, was just trying to work out all the controls
