During "Diving Area" (the mission after Lara gets bonked) you'll eventually open the door to a helipad. Once you enter 3 thugs will spawn and the heli will leave.
Get rid of the enemies, stand somewhere in the room and do the following steps:
step forward, step back, circle 3 times (direction doesn't matter - you can overshoot a little), forward jump.
Voila, childhood trauma.

Tomb Raider II - Diving Area (Level 6)
After using 2 switches underwater you will enter a staircase hallway with a door that automatically opens. Ahead is a helipad, 3 enemies will spawn and chase you so take them out. When they're dealt with, approach the helipad and it helicopter flies away. Now enter the level skip cheat code from Tomb Raider 1:
Hold Walk, press Up for a step forward, press Down for a step back, use Left or Right to spin in a circle 3+ times, then hold Jump and Forward
If done properly, Lara will explode and you will get this achievement.
