This needs to be done on the second level, Venice.
Take the first boat (#1) you encounter up until the end of the level.
You don't need to take any keys for now, just leave the boat #1 on one side of the path to the mines at the end of the level.
Near the mines is the second boat (#2), you just need to take it and drive in the direction of the mines.
When you are near the boat #1 you left near the mines, hold JUMP and the direction you left the boat #1 on the D-Pad, so Lara can jump off the boat #2.
You need to do that really near the first boat's seat, so she can leave the boat #2 and sit on the boat #1 seat.

Oddly, I got this achievement by simply jumping out of the boat before it hit the mines. I hadn't opened the door either, because I was using the non-timed sequence

Same for me, the achievement popped when I simply jumped from the boat into the water. That was before I opened any of the gates and I used the second boat forb that, directly after boarding it for the first time.

Tomb Raider II - Venice (Level 2)
After taking the first boat into a dark tunnel, you will find a boat lift you can activate to bring it into the next area. From here, take the left path and smash through the gondolas. Forward at the end of the canal are some mines, and an enemy appears on the left dock.
Jump onto the dock to your right to kill the enemy. Then grab the other boat there and park it near the mines, lining up Lara with the middle/edge of the windows on the walls. I found it best to have the boat neither up against the walls or in the dead center of the canal, but in the middle of those two options. Now swim to the other boat, make a save and line it up to jump out (hold Jump + left or right) at the last second into the other one.

Ever since patch #2, the achievement only seems to unlock when properly meeting the requirements.
